Unusual Presentation of Guillain-Barré Syndrome Following COVID-19 Vaccination

Case Presentation:

A previously healthy 34-year-old male presented to the emergency department with complaints of bilateral facial weakness and progressive ascending weakness in all extremities developing over 72 hours. Symptoms began 10 days after receiving the second dose of an mRNA COVID-19 vaccine. The patient denied recent illness, travel, or unusual exposures. Neurological examination revealed bilateral facial palsy, areflexia in all extremities, and symmetric ascending weakness (3/5 strength in lower extremities, 4/5 in upper extremities). Sensation to light touch and pinprick was preserved. Initial vital signs were within normal limits. Laboratory studies showed normal complete blood count, metabolic panel, and inflammatory markers. Cerebrospinal fluid analysis revealed albuminocytologic dissociation with protein 87 mg/dL (normal 15-45 mg/dL) and white blood cell count 2 cells/μL (normal 0-5 cells/μL).

Diagnostic Assessment:

Nerve conduction studies performed on day 3 of admission demonstrated prolonged distal motor latencies, conduction block, and absent F waves in multiple nerves, consistent with an acquired demyelinating polyneuropathy. MRI of the brain and spinal cord showed no significant abnormalities. Extensive infectious workup including PCR for Campylobacter jejuni, cytomegalovirus, Epstein-Barr virus, HIV, and SARS-CoV-2 were negative. Anti-ganglioside antibody panel was positive for GD1b IgG antibodies. Based on clinical presentation, CSF findings, and electrophysiologic studies, the patient was diagnosed with Guillain-Barré syndrome (GBS), specifically the facial diplegia with paresthesias variant with atypical ascending progression.

Our literature review identified only 17 previously reported cases following mRNA COVID-19 vaccination, with an estimated incidence of 1 per 10 million doses.\n\nThis case is particularly noteworthy for three reasons: First, the patient presented with bilateral facial weakness as an early and prominent feature, consistent with the facial diplegia variant, which represents only 3% of GBS cases. Second, the progression was more rapid than typical for this variant. Third, the presence of GD1b antibodies, which are more commonly associated with sensory ataxic variants of GBS, represents an unusual immunological profile for this clinical presentation.
